information so that your Order will not be delayed:


1) Your birth data ... date, time, place AND birth data of a related person
(if applicable to the reading).

3) Complete the SHIPPING section of the order form if you wish your tape
mailed (by USPOSTAL SERVICE) to a DIFFERENT address.
